Anatomy

The largest and highest of the three cuneiforms, between navicular behind and first metatarsal in front — it carries the great-toe column and most of the push-off load in stance. This is the left member of the pair.

Clinical notes

Its avulsion fracture in direct twisting trauma is rare but deranges the medial arch; CT is needed since plain films hide it behind overlap.
