Anatomy

A long strap from the transverse processes of the upper cervical vertebrae to the scapular superior angle; its upper half pierces trapezius, creating the classic neck-shoulder pain crossover.

Origin & insertion

Posterior tubercles of the transverse processes of C1–C4.

Action

Elevates the scapular superior angle rotating the glenoid downward; with the girdle fixed it assists same-side neck flexion.

Innervation

Dorsal scapular nerve (C5) with direct branches from C3–C4.

Clinical notes

Overactivity in desk work and stress makes it the classic "tense neck" muscle; its trigger point under the superior angle mimics upper-limb radiation.
