Anatomy

A broad, prominent prevertebral muscle situated on the anterior surface of the upper cervical spine, lying lateral to longus colli. It arises by tendinous slips from the anterior tubercles of the transverse processes of the third through sixth cervical vertebrae, ascending obliquely medially to insert into the basilar part of the occipital bone. It acts as a powerful flexor of the head and upper cervical spine and assists in head rotation.

Origin & insertion

From the anterior tubercles of the transverse processes of the third, fourth, fifth and sixth cervical vertebrae (C3–C6), into the inferior surface of the basilar part of the occipital bone.

Action

Flexes the head and upper cervical spine; slightly rotates the head to the ipsilateral side.

Innervation

Anterior rami of C1, C2 and C3 cervical spinal nerves.

Clinical notes

Forms part of the muscular bed on which the retropharyngeal space rests; retropharyngeal abscesses or hematomas lie directly anterior to the longus capitis and prevertebral fascia.
