Pyramidal Decussation (Decussatio pyramidum)
Educational content only — not medical advice.
Anatomy
The pyramidal decussation is the crucial motor crossover zone located in the anterior lower medulla oblongata at the cervicomedullary junction. Here, approximately 85–90% of descending corticospinal motor fibers cross the anterior median fissure obliquely to form the lateral corticospinal tract in the contralateral spinal cord, establishing voluntary motor control of the opposite side of the body.
Clinical notes
Explains the fundamental neurological rule of contralateral motor deficits: cerebral stroke or internal capsule lesions rostral to the decussation cause hemiparesis on the contralateral side of the body.

FAQ
What is the role of the Pyramidal Decussation?
The pyramidal decussation is the crucial motor crossover zone located in the anterior lower medulla oblongata at the cervicomedullary junction. Here, approximately 85–90% of descending corticospinal motor fibers cross the anterior median fissure obliquely to form the lateral corticospinal tract in the contralateral spinal cord, establishing voluntary motor control of the opposite side of the body.
Why is the Pyramidal Decussation clinically important?
Explains the fundamental neurological rule of contralateral motor deficits: cerebral stroke or internal capsule lesions rostral to the decussation cause hemiparesis on the contralateral side of the body.
